Acti-Kare of the Blue Ridge has a service called the Early Planning Service. The goal of the early planning service is to work with local Hospices to have a conversation early in the hospice process with the families of their patients. Many families are so preoccupied with all the concerns and changes that occur when a loved one enters hospice. Often caring for their loved one on their own. Early on they handle the day to day care, but as time passes they find themselves getting overwhelmed by it all. Acti-Kare hopes to get to the families early in the process so they can have the services they need in place when they need them.
